We want to provide you with all the resources you need to help you get started. Below, we’ve listed a description of the resource with a link to access an example. Feel free to use these forms in your daycare or create one of your own!

Child Care Licensing Information by State: Every state has different licensing requirements. This will help you figure out exactly what you need to do in your area in order to get properly licensed.

Contract Form: This is where you state your hours of operation and what you expect to be paid.

Emergency and Health Information: You’ll need the child’s health information and the name of their doctor and dentist in case of emergency.

Transportation Form: A transportation form is necessary if you plan on taking the kids of your property. The parents need to sign a form that gives you permission to do so.

Child “Get to Know” Form: Get to know the children in your daycare by having their parents fill out this form.
